Referrals
If you or anyone you know would like to come for an assessment at the centre for physiotherapy or any of our other treatments, you will first need to send us a letter of referral from your GP or Consultant, confirming diagnosis of a neuromuscular condition and referring you for treatment.
Please ask your GP or Consultant to include your contact phone number in this letter so we can contact you.

Alternatively, a Specialist Neuro-Muscular Physiotherapist or a NeuroMuscular Care Advisor an complete our referral form and send it to reception@mdsupportcentre.org. The referral form can be found here –
Please contact us to let us know you have requested a referral. Once we have received it, we will arrange an appointment for your initial assessment.
